In a viral TikTok video, Nicholas Romano, a physician assistant student from Med Explained 2 You, explained that peeing before driving could potentially increase your chances of survival in the event of an accident. Romano compared the bladder to a balloon, stating that when you empty your bladder, it deflates the balloon-like organ; however, holding it in has the opposite effect. An inflated bladder is more susceptible to rupturing during a car accident, even if you are wearing a seatbelt. If left untreated, a ruptured bladder can lead to severe complications such as infection, sepsis, kidney failure, and even death. Romano emphasized the importance of deflating the bladder by emptying it before getting into the car, as it decreases the likelihood of it bursting. Although bladder rupture is relatively rare and typically caused by trauma, such as a car accident, it is crucial to be aware of the symptoms, which include blood in the urine, difficulty urinating, weak urine stream, painful urination, fever, and severe lower back pain. The Urology Care Foundation advises wearing a seatbelt across the lap rather than the abdomen to minimize the risk of rupture during a car crash. Romano reassured viewers that in the event of rupture, an extra-peritoneal abdominal rupture, which is less severe, is more likely to occur. This type of rupture involves a tear at the bottom or side of the bladder, causing urine to leak into the tissues surrounding it instead of the abdomen. Although rare, cases of bladder rupture have been documented, including one involving a 58-year-old man who was intoxicated during a car accident but survived after emergency surgery, and another involving a 30-year-old woman who experienced bladder trauma following a car crash. TikTok users expressed their shock and related personal experiences in response to Romano’s video.
