If you’re struggling with a consistently hoarse voice, you might be experiencing laryngopharyngeal reflux (LPR), commonly known as “silent reflux.” This condition occurs when stomach contents flow into the throat or voice box. LPR is less known than gastroesophageal reflux disease (GERD), but it shares similarities.
To understand this under-recognized syndrome better, we spoke to Babak Firoozi, M.D., a gastroenterologist at MemorialCare Orange Coast Medical Center, and Kevin Leahy, M.D., Ph.D., an associate professor of Clinical Otolaryngology and part of the Voice Center at Penn Medicine.
LPR typically presents with symptoms such as hoarseness, a sensation of a lump in the throat, throat clearing, chronic cough, and difficulty swallowing. It’s crucial to note that hoarseness may not always indicate LPR, as this symptom can be attributed to various reasons.
Diagnosing silent reflux often involves a flexible laryngoscopy or upper endoscopy, and doctors may also conduct an esophageal pH test or an esophageal manometry test to measure muscle activity. Treatment options revolve around medication to suppress stomach acid production, lifestyle modifications, and in severe cases, minor surgery.
Preventative measures for LPR include avoiding acidic and spicy foods and drinks, eating dinner earlier, and sleeping with an elevated head. If you’ve been experiencing hoarseness for more than four weeks, it’s time to see a doctor. Occasional hoarseness is normal, but prolonged symptoms need medical attention.
