GP Receives Six-Month Suspension for Failing to Schedule In-Person Appointment; Patient Hospitalized with Heart Infection

A GP, Dr. Alyn Williams, has been suspended after neglecting to arrange face-to-face appointments for a patient who was later hospitalized with a heart infection. The patient’s condition worsened significantly, leading to hospitalization.

The medical tribunal panel suspended Dr. Williams for his inadequate care and failure to meet expected standards. They concluded that the gravity of the patient’s illness may have been prevented if Dr. Williams had provided necessary and adequate care. Dr. Williams cited concerns over COVID-19 as the reason for not conducting face-to-face consultations, as he was considered vulnerable to the virus due to his age and obesity.

The patient, a long-term registered individual with Chapel Street Surgery, initially experienced symptoms of a persistent chesty cough and nasal drip during the height of the COVID-19 pandemic in May 2020. Dr. Williams prescribed a nasal spray during a telephone consultation. When the symptoms persisted, the patient was referred for a whole body CT scan and prescribed antibiotics after another telephone consultation. However, the patient’s symptoms continued, and blood tests conducted in December revealed high levels of CRP, indicating inflammation.

During subsequent telephone consultations, the patient complained of feeling run down, experiencing weight loss, and lacking energy. However, Dr. Williams again prescribed antibiotics without conducting further assessments. The patient’s condition deteriorated, and he was ultimately admitted to the hospital, where it was discovered that he had lost a significant amount of weight in a short period due to endocarditis, an infection of the heart’s inner lining and valves.

During the tribunal, medical expert Dr. C pointed out that a face-to-face assessment could have revealed additional symptoms such as a high temperature and emphasized the importance of monitoring the patient’s vital signs. Dr. Williams defended his decision not to conduct in-person appointments, citing limited available rooms due to building work and the need to keep patients isolated. He also claimed that the patient expressed a strong reluctance to go to the hospital due to concerns about his wife’s care and the risk of COVID-19 exposure.

Despite Dr. Williams’ justifications, the Medical Practitioners Tribunal Service determined that he should be suspended. They concluded that he repeatedly failed to arrange a face-to-face appointment and refer the patient to the hospital, and it was likely that the patient would have gone to the hospital if advised to do so. The tribunal criticized Dr. Williams’ conduct and contradictory evidence, noting his poor record-keeping and failure to document recommendations for hospital visits or the patient’s refusal.

While Dr. Williams attended a record-keeping course and had no prior complaints or referrals against him, the tribunal found his actions to have inadequately protected the patient and undermined public confidence in the medical profession. The suspension serves as a message to the profession and the general public about the importance of providing proper care and maintaining trust in the healthcare system.
