The National Transportation Safety Board (NTSB) has completed its investigation into a United Airlines flight incident that occurred in 2022. According to CNN, the NTSB investigators have determined that pilot error was the cause of the incident. The captain had called for the Boeing 777’s flaps to be reduced to a setting of 5, but the first officer misheard and thought the captain said 15. This miscommunication led to the plane descending and decelerating, causing alarms to sound. Fortunately, the pilot was able to pull the plane out of its nosedive.
